You must notify the plan administrator, in writing, of any employee’s qualifying event within 30 days of the qualifying event.

You must notify qualified beneficiaries currently receiving continuation coverage of their ability to continue coverage under a new group benefit plan for the balance of the period the beneficiaries would have remained covered by the prior group benefit plan if the beneficiaries’ coverage under the prior group benefit plan was set to terminate before the end of the period the qualified beneficiary would have remained covered.

This notice must be provided either 30 days prior to the termination or when all enrolled employees receive notification, whichever is later. Plan administrators must provide you, upon written request, information necessary to assist you in giving this notice. Then you must notify the successor plan in writing of the qualified beneficiaries currently receiving continuation coverage so that the successor plan, contracting employer or administrator, may provide those qualified beneficiaries with the necessary premium information, enrollment forms and instructions.

A plan can contract with you to perform these required notifications and collect premiums. If you have agreed to an arrangement, you must notify any qualified beneficiary who has elected continuation coverage of the date that their coverage will terminate and notify the qualified beneficiary of any conversion coverage available. You must provide this notice during the 180-day period ending on the date that continuation coverage is terminated. This does not apply when the continuation coverage is terminated because the group contract between the plan and the employer is being terminated.
